"NOTIFICATION OF INTENT TO GRADUATE" FORM

Students must inform the Department of Music in writing of their intent to graduate by submitting a Notification of Intent to Graduate form.

deadline: This form must be submitted to the Graduate Coordinator by the FIRST day of the semester BEFORE the semester that you intend to graduate.
